If you have children attending any of our Summer Camps, we require you to complete the following forms before your child is permitted to be left in our care. Forms submitted prior to March 2016 have been securely discarded. We ask that you complete new forms to ensure our files are accurate and up to date.

Required for all Summer Camp participants. Includes Transportation Form, Medical Information and Permission Slip Form.

Pre-order your child’s lunch, and it will be delivered to them at camp.
